Account Deletion

• In the app: sign in, open Help & Support, and choose Request account deletion. You will be asked to confirm before the request is sent.
• By email or phone: contact us at support@hsim.org or +1 360-888-3129 from the email address on your account, and say you would like your HSIM account deleted.

HSIM will review your request, may verify your identity, and will contact you about the outcome.

When a request is approved, we will remove or deactivate your sign-in account and profile details that we are not required to keep.

Some information may need to be kept where required or permitted by healthcare, employment, payroll, financial, legal, security, audit or other applicable obligations — for example patient care records, visit and mileage records, payroll records and audit logs.
